In response to the DSM-IV question, here are a few indices containing DSM-IV codes...

this site also contains a numerical listing; the file designation is /dsm4n.html

Additionally, there is a Canadian site at http://www.mentalhealth.com/p.html that has DSM-IV and ICD-10 diagnotic criteria (including self-diagnostic applets) for many common Axis I and II disorders.
