It seems to me that the good doctor is not really treating her. I would have expected him give her some assertiveness training a long time ago, perhaps in the form of a role-playing exercise or two.

I'm beginning to wonder if the doctor is subconsciously not treating Anna so she'll keep coming to see him. This would make for a very interesting plot development. Whichever way the author is going with this, the time has come to do something one way or the other. Either begin assertiveness training, or have Anna challenge the doctor about lack of treatment.
