I don't know about making the actual session transcripts RealAudio, but it might work if it were done with only the Doctor's Notes. Doctors normally dictate to themselves using a mini-recorder regarding patient visits...that would seem more realistic.

As for this going to television, it's already been done, in a way. There's an animated show on Comedy Central called "Dr. Katz, Professional Therapist" that is simply hilarious (a good reason, I gather, for it to be on Comedy Central).
