Patient Name: Sylvia Bows

Year of Birth: 1958

Occupation: Senior Vice President in Charge of Customer Service, SII

Original Complaint (physical): Rapid pulse and respiration, abdominal involvement, dizziness and insomnia

Original Complaint (psychological): Anxiety attacks, murderous rage

Brief Summary: Ms. Sylvia Bows is a high ranking executive at SII. She had been trying to get pregnant for two years and suddenly found out why she had failed: her husband had undergone a secret vasectomy. After she was able to control her homicidal rage, Sylvia plots a methodical plan to revenge herself on her husband and to get what she wants. Total number of sessions: 53.

Brief Impression: Very passionate but very controlled. Highly intelligent and articulate and very successful in her career, Sylvia feels that she has been betrayed by her businessman husband, Tom. While they were trying to conceive a child, Tom had a secret vasectomy. Sylvia has lost all sense of loyalty to her husband and felt out of control of her life, a new experience. She has been engaging in a series of extramarital affairs, perhaps encouraged due to an unanticipated side effect to Zoloft.

Tentative Diagnosis: Bipolar disorder, depression, possibly recurrent, brought on by personally catastrophic event.

Current Medications: Sinequan increased from 75 mg to 100 mg on 10/4/96. Zoloft (Sertraline, a serotonin reuptake inhibitor) discontinued 9/3/96.

Current Status: Sylvia is experiencing a post-traumatic reaction to the 48 hour kidnapping of one of her twins.
